DESCRIPTION:

Mutual of Omaha is the college's provider of life insurance products.

ELIGIBILITY:

All full-time employees are eligible.

ENROLLMENT:

Enrollment is upon hire to a full-time position.  Coverage becomes effective the first of the month following 60 days of employment.

BENEFITS:

The following benefits are paid for by your employer:

LIFE and AD&D

$20,000 Basic Life Insurance and AD&D (Accidental Death & Dismemberment)

Benefits will reduce as follows:

• 35% upon the attainment of age 65
• An additional 25% of the original amount at age 70
• An additional 15% of the original amount at age 75
• Benefits will terminate upon retirement

At the insured person's death, the amount of life insurance will be paid to the surviving beneficiary.

Long Term Disability Insurance

Benefit amount is equivalent to 60% of your before tax monthly earnings, not to exceed the maximum monthly benefit of $5,000.

The policy does have a pre-existing exclusion.
