Equipment Booking Form

This form is for submitting equipment booking requests to the library. The library staff will do their best to accommodate your needs given the supply of equipment available. Once booked, unless you are notified otherwise, your equipment will be available for you to pick up at the requested time at the front desk of the library.

All fields are required.
