Release of Records

The Admissions & Records Office strictly enforces the Family Educational Rights & Privacy Act (FERPA).

Student records will not be released without the written consent of the student.
If you would like someone else to have access to your records, click here to access a Consent to the Release of Academic Information form. This form must be on file in the Admissions & Records Office before any information can be released.
What information must I include in the consent form?
Either print and fill out the consent form, or include the following information in your request:
- Print your first name, middle initial, and last name
- Social Security Number
- Designate what information you would like shared: Grades, Academic Progress, Attendance, Class Schedule, Financial Aid Information, Payment Information
- Print the name of the individual(s) and their relationship to you of who you are giving access to this information
- Designate the deadline of how long this individual(s) has access to this information.
- Sign form
Phone or e-mail requests for release of records are not accepted.
